It happened to me a few times on my VFC 416. I tried a King arms lever and it worked but the geometry of the lever was a bit off from the stock one the detent ball didn't line up with the dimples in the body.

I mixed the VFC lever with the KA disc, had to press them together. Then I made sure I wound the fixing screw on tight. It has been solid since then (2+ years).
