They fail miserably because a LOT of the blast comes from the gap between the cylinder and the barrel. On a small caliber revolver, it may help a bit for a little while. On a military revolver that will get battered it's useless. On a .44? It's a total waste of time. There is enough side blast with a .44 to blow your hands off.

(Yes, I own a real M29, before anyone asks).

Movies use anything that looks cool, even if it does not work.
